We offer a repeat prescribing service for patients we have seen before and medications we have previously prescribed.

There is a £10 charge for this service (plus the cost of a 1st class stamp if required) and we will send you payment details on receipt of your prescription request.

Please fill in the form below and remember to let us know which option you would like :

• have the medication delivered to you by our online partner pharmacy (they will contact you for payment)
• have the prescription emailed to you for you to take to a pharmacy of your choice
• arrange a time to pop in to pick up the prescription or
• have the prescription posted to your registered address (and you will pay for the medication at your chosen pharmacy).
